An allocated 45 minutes at start of medical is dedicated to worker completing relevant medical/personal history. As a service to employers/companies this questionnaire can be downloaded and completed prior to appointment. This requires the employee to present 30 minutes prior to appointment time.

We do request that workers present at the time delegated by surgery staff to avoid any inconvenience to other surgery clientele.

Completing the form will require 15-30 minutes of your time.

NB:- All questions (including 3 previous positions held) must be included in the history - if relevant.
